It appears to be the earliest pharmacopoeia of Ayurveda. The title is mentioned in the folio 12a, 16th line as
Beginning:
DhanvantarInighantuh (इति धन्वंतरीये (य) निघंटे (टी) गडूच्यादिप्रथमो (S) - ध्यायः II) But the cover page contains the title as Dravyāvalināmanimayah The text continues upto कृतान्नवर्ग:, the manuscript is incomplete at the end. Author's name is not mentioned here, but Gurupada Halder in his Vaidyakavṛttanta ascribes the authorship to Dhanvantari, a contemporary of Amarasimha, Prof. P.V. Sharma however, in his edition of Dhanvantari-Nighantuḥ asserts that 'Dhanvantari' is not connected with the authorship of this work.'

श्री परमात्मनि धन्वंतरये विश्वभेषजाय जगद्धेतवे ॥
नमानि (मि) धन्वतर (न्तरि) मादिदेवं सुरासुरैर्व (र्व) दितपादपद्यं (धम्) ।
